Director, Population Health Impact, Remote at Aledade
This Director of Population Health will steward the development and implementation of our healthcare equity commitment to delivering equitable clinical outcomes and transforming the healthcare experience for all. This Director will partner with cross functional teams throughout the organization to meet and exceed our goals to decrease healthcare disparities and realize health equity in the communities we serve.
The Director of Population Health will be the leader responsible for the investigative data analysis, solutioning, and execution of key initiatives and projects and ensure they meet our clinical and financial healthcare equity goals and drive measurable continuous improvement. They will collaborate with other leaders to develop and implement processes, pilots, and partnerships to reduce healthcare inequity and disparities for under-resourced and historically marginalized communities.
