Paramedic, Acute Care at Home at Essen Medical Associates

Job Summary

Bring Acute Care to the Patient

Essen Health Care is building an Acute Care at Home program that delivers timely, high-quality acute care to medically complex adults in their homes. Our physician-led team combines paramedics, clinicians, care coordinators, remote monitoring, and community-based services to provide the right care at the right time.

This is not a traditional 911, emergency response, or patient-transport role. As a Paramedic on our Acute Care at Home team, you will bring strong clinical assessment and IV skills directly to patients, working under established protocols and real-time medical direction.

Transportation between patient visits is provided by a dedicated driver—no personal vehicle is required.

Responsibilities

What you’ll do

  • Provide in-home acute assessment and treatment under approved protocols, standing orders, and medical direction.
  • Perform focused clinical assessments, including vital signs, symptom review, medication reconciliation, cardiopulmonary assessment, and evaluation for clinical deterioration.
  • Establish and maintain peripheral IV access; administer ordered IV fluids and medications.
  • Obtain laboratory specimens, initiate remote patient-monitoring equipment, and document care accurately in the electronic medical record.
  • Support treatment plans for appropriate acute conditions, including dehydration, infection, cellulitis, respiratory symptoms, and heart-failure exacerbation.
  • Communicate clinical findings and treatment response in real time with the supervising physician, advanced practice provider, and interdisciplinary care team.
  • Follow defined escalation pathways and activate emergency services immediately when a patient requires a higher level of care.
  • Educate patients and caregivers on treatment plans, medications, warning signs, and follow-up needs.
  • Maintain field equipment, supplies, and clinical readiness.

Qualifications

What you bring

  • Current, unrestricted New York State Paramedic certification.
  • Current BLS and ACLS certifications.
  • At least two years of recent paramedic experience in 911, emergency medicine, critical care transport, mobile integrated health, community paramedicine, or urgent care.
  • Strong IV-access, medication-administration, acute-assessment, and clinical-escalation skills.
  • Comfort working independently in patients’ homes while staying closely connected to a remote clinical team.
  • Ability to use mobile technology, electronic medical records, and remote-monitoring tools.

Preferred: Experience in home-based care, community paramedicine, mobile integrated health, emergency department, critical care transport; bilingual English/Spanish or other language skills relevant to the communities we serve.

Work Environment

This is a mobile, field-based role serving patients in their homes throughout the New York City metropolitan area. You must be able to safely perform in-home clinical assessments and treatments and move and set up clinical supplies and equipment.
