TGH Hospital at Home is a distinct and innovative hospital medicine service at Tampa General, designed to facilitate the transfer of hospitalized patients to a home setting for the continuation of their inpatient care. Within the Hospital at Home unit, patients benefit from a minimum of two in-person nursing visits, along with a daily provider visit, which may be conducted virtually or in person. Advanced Practice Providers (APPs) play a crucial role in screening for eligible Hospital at Home patients within TGH’s inpatient and emergency room environments. The clinical responsibilities of APPs include, but are not limited to, daily patient rounding via telehealth (and may be required to perform in-person assessment at a patient’s home when clinically necessary), admissions, discharges, and consultations. The APP will contribute to enhancing patient care and operational efficiency. APPs are empowered to make autonomous medical decisions, allowing them to provide a comprehensive range of diagnostic and therapeutic services.
The APP role encompasses participation in quality and process improvement initiatives, ongoing program evaluation, and the development of strategies for continuous improvement. The acute care Hospital at Home APP will collaborate closely with their Supervising Physician and multidisciplinary teams, as well as the hospital-based consulting services. The APP will report directly to the Manager of Advanced Practice Medical Services.
Working Conditions
Work is performed primarily at TGH main campus and at Corporate Center office. Requires travel between TGH facilities and travel to patients home when necessary. Shifts include: 4 – 10 hr shifts (Day) with at least 1 weekend per month and rotating remote after hours on-call 1-2x/week (subject to change pending staffing needs). Daily interactions with multidisciplinary TGH team members, Shields at Home team members, physicians, consultants, patients, family members, and occasional external parties (media, government officials, public).
